Third Medical Opinion?

An minor ongoing medical issue prompts me to post this experience:

Medical Problem - A small common cyst on a finger for about two years which disfigures the nail and looks like it could burst. Gets in the way of filing and some other office work as well as gardening, aquarium and koi pond maintenance work. Patient has a history of a hospital aquired MRSA.

Surgeon # 1 Takes an x-ray
Plan of care: Outpatient surgery with IV sedation, pre-op testing required. Vancomycin to prevent MRSA will be given by IV just prior to and during surgery.
Three weeks until sutures are removed. Surgery in the joint from where the cyst arises may cause some slight permanent stiffness.

Surgeon # 2 Does not review the x-ray. Says and I quote "I know exactly what's going on in there without an x-ray".
Plan of Care: The surgery will be under IV sedation with pre-operative testing required. Will not administer prophylactic antiobiotics. Requires patient to visit an infectious disease doctor for full communicable disease workup and "plan" prior to scheduling the procedure. A skin graft will be necessary to cover the area of the incision. The finger will be splinted for three weeks. Finger physical therapy (?????) may be necessary to regain full use of the digit.

Surgeon # 3 Reviews the original x-ray and orders an additional view taken.
Plan of Care: Surgery with local anesthetic. Patient to take oral Clindamycin on the day prior to, day of and day after surgery for infection prevention. Stitches will be out in twelve days. No permanent stiffness expected.

I chose Surgeon #3 for obvious reasons. The offending digit is on the right hand middle finger. That is the finger I would like to give to surgeon #2.

It pays to shop around. I can just imagine what the co-pays and out of pocket expenses would have been had I chosen surgeon #2. All unnecessary. Talk about running up the cost of health care. This guy's the poster child for runaway health care expenses.

have you had the surgery? if so and your out come is as #3 expected you made a good choice. If you haven't had the surgery I hope #3 is correct, but if one or 2 were correct then you may well have chosen poorly. Sometimes getting the answers that we want, is not always the anser that serves us best. In any event I hope your surgery goes well if it has not already taken place.

You're right about the choice we want may not be the best choice. No I havn't had it done yet. I'm trying to hold off until September. Sooner might be necessary. I had done some previous research (WebMD etc.) and understood the basics of the problem and the usual treatment. The obvious out was surgeon #2. The real choice was between #1 and #3. I did choose #3 because I wanted to avoid the IV sedation along with the additional out of pocket costs. Also a "communicable disease workup" was absolutely not necessary. I confirmed this with my family physician. My thinking is that if #3 finds more than he expected I will have the outcome of #1 but easier on my wallet.

Fourth opinion:

1. Drink heavily.
2. Stick finger in ice water for 10 minutes.
3. Drink some more.
4. Grab tweezers.
5. Grab dry ice.
6. Drink some more.
7. Say prayer.
8. Stab cyst with dry ice.
9. Holler like a cat in a bonfire.
10. Drink some more, congratulate self for performing surgery for the low price of a fifth of cheap vodka and a chunk of dry ice you mooched from the ice cream man.

I would do the legwork aka research yourself as well. I'd recommend that for everything. Passive decision making tends to get passive results and tend to reinforce professional incompetency as the status quo.

Getting second opinions pays off. One of the major problems in healthcare is taht patients refuse to take the initiative and seek out different opinions. With a few exceptions, medicine is not a purely scientific enterprise that tells to physician to do X then Y then Z. It's an art that says get to Z the way you think is best.

My anesthetic of choice is Jack Daniels. I've numbed a lot of stuff with that over these many years.
